• When you make a request to Mentor Patient Services through "Ask Mentor", we will collect your name and e-mail address;
• When you purchase an enhanced warranty through the Web site, we will collect your name, address, telephone number, e-mail address, birth date, gender, zip code, Social Security number, driver’s license, and credit card payment information. Your surgeon’s name and address, along with your date of surgery and implant reference number, will also be collected.
Mentor will use your personal information only for the purpose indicated to you when you provided it. Generally, our use of such information is limited to the following:
• to respond to your requests;
• to develop records with regard to your interest in, purchase or use of Mentor products or warranties;
• to contact you with information that might be of interest to you, including information about products of ours and investor relations information;
• to enforce this Privacy Policy and the Terms of Use of this Web site;

• to comply with a law or regulation, including government investigations, validly issued subpoenas, and court orders.
In the ordinary course of business, we will share some personal information with companies that we hire to perform services or functions on our behalf.
